  1. Office of Statewide Health Planning and Development (OSHPD), Long-term Care Facility Financial Data: Report available as of 7/23/2009.

  2. The California Department of Public Health,
       includes Citations received by CANHR as of 12/2/2009;
        Enforcement data (other than Citations) has not been received from the State since 4/30/2008.

  3. The Centers for Medicare and Medicaid Services (CMS), Online Survey, Certification, and Reporting (OSCAR) data, as of June 2006.

  4. The Centers for Medicare and Medicaid Services (CMS), Region IX, federal enforcement data report generated July 2006.

  5. California Department of Health Care Services (DHCS), Medi-Cal Statistics Section.

  6. California Advocates for Nursing Home Reform (CANHR) questionnaires, completed and returned by facilities. Most recent received.

    Note: "CANHR-tracked facilities" are very close to the set DHS refers to as SNF/NF (which excludes ICF-DD facilities), differing only by a few facilities.The omitted facilities specialize in residents with acute mental conditions or do not accept residents from the general population (e.g. correctional facilities).
