  2. The California Department of Public Health, Data as of 2/25/2008; citations received by CANHR as of 4/1/2008.

  3. The Centers for Medicare and Medicaid Services (CMS, formerly the Health Care Financing Administration(HCFA)), Online Survey, Certification, and Reporting (OSCAR) data, as of June 2006.

  4. The Centers for Medicare and Medicaid Services (CMS, formerly the Health Care Financing Administration(HCFA)), Region IX, federal enforcement data report generated July 2006.

  5. California Department of Health Services (DHS), Medi-Cal Statistics Section.

  6. California Advocates for Nursing Home Reform (CANHR) questionnaires, completed and returned by facilities. Most recent received.

    Note: "CANHR-tracked facilities" are very close to the set DHS refers to as SNF/NF(ICF) (which excludes ICF-DD facilities), differing only by a few facilities.The omitted facilities specialize in residents with acute mental conditions or do not accept residents from the general population (e.g. correctional facilities).
