  1. Office of Statewide Health Planning and Development (OSHPD), Long-term Care Facility Financial Data: most recent reports issued as of April 2010.

  2. The California Department of Public Health,
       includes Citations received by CANHR as of 7/16/2010;
        Name, Complaint & Ownership data as of 5/24/2010. [Note: Deficiency information last received 9/30/2008]

  3. The Centers for Medicare and Medicaid Services (CMS), ASPEN (Automated Survey Processing Environment) , data has not been received since 9/30/2008

  4. The Centers for Medicare and Medicaid Services (CMS), Federal enforcement actions data received by CANHR as of 04/13/2010.

  5. California Advocates for Nursing Home Reform (CANHR) questionnaires, completed and returned by facilities. Most recent received.

    Note: "CANHR-tracked facilities" are very close to the set DHS refers to as SNF/NF (which excludes ICF-DD facilities), differing only by a few facilities.The omitted facilities specialize in residents with acute mental conditions or do not accept residents from the general population (e.g. correctional facilities).
