The information in this web site is based on data collected from the following sources:
  1. Office of Statewide Health Planning and Development (OSHPD), Long-term Care Facility Financial Data: most recent reports issued as of April 2010.

  2. The California Department of Public Health,
       Includes Citations received by CANHR as of 1/26/2012;
        Enforcement data from the State as of 8/30/2010 (though incomplete)

  3. The Centers for Medicare and Medicaid Services (CMS), ASPEN (Automated Survey Processing Environment)

  4. The Centers for Medicare and Medicaid Services (CMS), Federal enforcement actions data received by CANHR as of 04/13/2010.

  5. California Advocates for Nursing Home Reform (CANHR) questionnaires, completed and returned by facilities. Most recent received.

    Note: "CANHR-tracked facilities" are very close to the set DHS refers to as SNF/NF (which excludes ICF-DD facilities), differing only by a few facilities.The omitted facilities specialize in residents with acute mental conditions or do not accept residents from the general population (e.g. correctional facilities).
